Application for Funding Assistance

Muscular Dystrophy Canada administers the medical equipment program under protest because we consider it a responsibility of governments to meet from public resources the legitimate equipment needs of Canadians who have disabilities.

The MAXIMUM AMOUNT PAID for any item is $2,640.

A $200 deductible will be applied to all funding requests up to $2640. If the single item cost exceeds $2640 and additional funds are required the $200 deductible may be waived at the discretion of the Director of Services.

NO items costing under $200 will be funded through Muscular Dystrophy Canada.

FUNDING REQUESTS All requests for funding of core equipment must contain a vendor’s quote and a prescription from a qualified health-care professional. (The prescription may be waived if the requested item is to replace an existing prescribed item when the user’s requirements have not changed.)

Muscular Dystrophy Canada does NOT provide reimbursements.

• Ramps/Platform Lifts (primary access to the home only)
• Ventilators / Cough Assist
• Lift Chairs
• Patient Lifts
